REGISTRATION OF X-RAY MACHINES

If you are a new facility with new equipment, you must submit an ARRA-4 Form with the appropriate supplementary machine forms.

If you take any actions which change the basic original information on the ARRA-4 Form you must re-apply with the correct information. Examples requiring re-filing of the ARRA-4 Form include, new address, change of ownership or a name change.

It is possible to make changes to an existing registration by filing a machine form only such as the ARRA-4X, ARRA-4IG or ARRA-4IR. The expiration date will remain the same as that shown on the previous registration. If your registration is near expiration, submit the ARRA-4 Form with the supplementary machine forms and a new expiration date will be generated and applied to your amended registration.

The supplementary ARRA-4X Machine Form is for use with the following disciplines: Hospital, Medical, Mammography, Podiatry, Veterinarian, Dental, Educational and Chiropractic. The ARRA-4IG and ARRA-4IR Forms are classified for industrial and some educational use.
