New Industrial Laser Registrants or Facilities with Change of Ownership

Complete:

• 1-ARRA-4NIR Form to register facility
• AZRRA-NIMI - Nonionizing Machine Information
  for EACH laser unit you are going to utilize
• Provide a signed LSO acknowledgment statement that designates the LSO, the LSO’s responsibilities, and the alternate LSO.
• Written explanation of intended use of the laser(s)
• Send in fee**

* Laser Operators must be trained in the assessment and control of laser hazards.
**Fees are determined according to Class A, Class B, and Class C categories. Please refer to the Registration Fee Schedule.

You will need to provide all the above to complete the process of your application. Do not leave any blank spaces, incomplete forms will be returned.

 

Current Registrants that want to Renew, Change Facility Name, or Change Address

Complete:

• 1-ARRA-4NIR Form to register facility

Current Registrants that want to make an Amendment to their Registrations, (Add/remove units, change list of operators, or change Certifying Official)

Complete:

• AZRRA-NIMI - Nonionizing Machine Information
  for EACH laser unit you are going to utilize
• No fee***
