Medical Radio Frequency (RF) Facility

New Medical RF Registrants or Facilities with Change of Ownership

Complete:

• 1-ARRA-4NIR Form to register facility
• AZRRA-NIMI - Nonionizing Machine Information
  for EACH RF device you are going to utilize
• Provide a signed RF acknowledgment statement
• Send in fee**

*Operators must be trained in the assessment and control of RF hazards.
**Fees will be $47.00 for each RF device.

You will need to provide all the above to complete the process of your application. Do not leave any blank spaces, incomplete forms will be returned.

 

Current RF Registrants that want to Renew, Change Facility Name, or Change Address

Complete:

• 1-ARRA-4NIR Form to register facility
• No fee***

 

 

 

Current RF Registrants that want to make an Amendment to their Registrations, (Add/remove units, or change rf safety Officer)

Complete:

• AZRRA-NIMI - Nonionizing Machine Information
  for EACH RF device you are going to utilize
• No fee***

If changing RF Safety Officier:

• Provide a signed RF acknowledgment statement
