New Industrial Radiofrequency Registrants or Facilities with Change of Ownership

• 1-ARRA-4NIR Form to register facility
• AZRRA-NIMI - Nonionizing Machine Information
  for EACH RF device you are going to utilize
• Provide a signed RF acknowledgment statement
• Send in fee**

*Radiation Safety Officers and Certified Operators must be trained in the assessment and control of RF hazards.
**Fees are determined according to Class A Industrial, Class B Industrial, and Class C Industrial RF categories. Please refer to the Registration Fee Schedule.

You will need to provide all the above to complete the process of your application. Do not leave any blank spaces, incomplete forms will be returned.

 

Current Registrants that want to Renew, Change Facility Name, or Change Address

Complete:

• 1-ARRA-4NIR Form to register facility
• No fee***

 

 

 

Current Registrants that want to make an Amendment to their Registrations, (Add/remove units, or change rf safety Officer)

Complete:

• AZRRA-NIMI - Nonionizing Machine Information
  for EACH RF device you are going to utilize
• No fee***

If changing the RF Safety Officer:

• Provide a signed RF acknowledgment statement
