New Tanning Registrants or Facilities with Change of Ownership

• 1-ARRA-4NIR Form to register facility
• AZRRA-NIMI - Nonionizing Machine Information for EACH tanning bed you are going to utilize
• Send in fee**

*Certified operators must meet the requirements stated in R12-1-1414 (est. Page 218)
**Fees will be $28.00 for each tanning bed. You will need to provide payment for the fee to complete the process of your initial application.

You will need to provide all the above to complete the process of your application. Do not leave any blank spaces, incomplete forms will be returned.

 

Current Registrants that want to Renew, Change Facility Name, or Change Address

Complete:

• 1-ARRA-4NIR Form to register facility

Current Registrants that want to make an Amendment to their Registrations, (Add/remove units, change list of operators, or change Certifying Official)

Complete:

• AZRRA-NIMI - Nonionizing Machine Information
  for EACH tanning bed you are going to utilize
• No fee***
