LASER TECHNICIAN CERTIFICATION
Initial Application $30.00 (Can have an unlimited amount of procedures)
Amendment to Application $30.00 (To add additional procedures, name/address changes, etc. after your original application has been processed)
Renewal Application $30.00 (Certificates will expire a year from issue date)
Duplicate certificates are $20.00 at any time. However, if a duplicate certificate is requested at the time of initial, amendment, or renewal application, the fee will be $10.00 per duplicate certificate.
